Well... this might be a continuity error... but both Mikhail and Ethan were breathing after they were presumed dead... it's just Ethan was buried (Nikki/Paulo Anyone?), and Mikhail wasn't.

I don’t think so as if that were true they wouldn’t be so bothered about these young women dying in childbirth and it would have meant Goodwin would have had to rush back to play dead after Ana-Lucia killed him then Jin and Eko came across him (although he did look to be in a different position) and Pickett wouldn’t have been so wound up by the Col’s death and Juliet wouldn’t have needed to get Jack to come try and help save Col if she knew the island was going to sort it
