Those are good caps Roco, and I agree that I reckon Libby came into being due to Mike's very guilty and very troubled sub-concious, and so he views certain aspects of his hospital stay as different (the monitor, the man).
The only other thing I saw and wondered if it had any meaning was the first monitor - the one in Mike's dream - having the words 'All Alarms Off' on it. My knowledge of hospital monitors isn't too hot, but I got the impression that it meant that if Mike deteriorated and needed assistance straight away, no nurse or doctor would know about it because they wouldn't hear an alarm. Of course it might not mean that, but maybe it suggests - in the context of Micheal's dream - that he doesn't deserve any more help - doesn't deserve an extension of life - if he gets close to death, due to him taking two lives himself. Given his suicidal state of mind, maybe he wanted such a thing: that if something went wrong when he was in hospital he wouldn't want to be resuscitated.
